Carta Onboarding

We're working to gather all of the necessary forms and documents that our implementations manager asked for and we have copies of our options agreement and exercise agreement that we've used for several employees, but we're having a hard time finding a blank form of these.

Can we just submit an example of the form that we've used for an employee? Or does it have to be a blank form? 

We do need a blank form of the options agreement and exercise agreement. These documents will live in Carta and be referenced by shareholders. Ensuring that the forms are blank allows your shareholders to view these documents and understand the terms of their equity.
