Company didn't survive: How do I get tax form from Carta for capital loss?

I bought shares of a company that unfortunately didn't survive.   On Carta the shares still show as holding a small value but I would assume the market value is essentially zero.    Is there a way I can get a tax document from Carta showing this as a capital loss?
