Cap Table Format

I'm working on gathering the information on for my company's cap table to ensure that it's all complete and accurate. My question is, does my cap table excel file need to be in a certain format? Or can I just send it over in the format that I've compile over the past several months? 

Want to make sure that I'm good to go! Thanks in advance.
